Job Summary:

Lowell Community Health Center is seeking a skilled Nurse Practitioner to join our team in providing high-quality healthcare services to our patients. As a Nurse Practitioner, you will be responsible for providing comprehensive care to individuals and families in an outpatient primary care environment. Your expertise will be invaluable in managing, providing, and evaluating nursing care in your assigned area of responsibility.

Responsibilities:
  • Assist with the provision of care that includes treatment, teaching, and counseling.
  • Recognize and interpret symptoms of patients' conditions and report to the appropriate healthcare provider.
  • Provide direct nursing services to patients and their families, including nursing assessment, intervention/referral, and healthcare education.
  • Assess patients' needs based on their current functioning, review of previous health/medical plans of care, and medications.
  • Prepare patients for examination, complete history, review systems, and current physical health status.
  • Take blood pressure, weight, height, temperature, and other measurements as appropriate to complete health assessments.
  • Perform vision and hearing screenings as necessary.
  • Obtain urine and sputum specimens as required.
  • Complete laboratory/radiology imaging reconciliation.
  • Administer appropriate immunizations and injections.
  • Maintain clinical logs for your assigned area.
  • Develop a system for summarizing clinical and laboratory data for evaluation and planning purposes, and quality assurance.
  • Assist with the planning and implementation of educational programs for patients and families to increase awareness of relevant medical conditions, decrease risk behaviors, and increase responsibility for self-care.
  • Attend and participate in in-service education programs for staff, quality assurance programs, and risk-management programs.

Qualifications:
  • Graduate of an accredited School of Nursing.
  • Registered Nurse License in the state of MA.
  • Current CPR/BLS certification.
  • Excellent interpersonal and communication skills.

Work Environment:

Frequent contact with patients, families, visitors, nurses, and other department staff. Occasional requests to change work areas to cover another priority area to meet patient needs. Occasional stress and pressure caused by multiple simultaneous demands. May be exposed to various body fluids and/or infectious diseases. Universal precautions and techniques are included in orientation, and the availability of protective wear is supported and encouraged when the employee is at risk for exposure.
