Work Schedule:
Typically, hours are Monday through Friday from 8:30 AM to 5 PM, with occasional extended hours for patient care. Schedules may vary based on operational needs.
Minimum Qualifications:
- Knowledge of nursing practices acquired through graduation from an accredited nursing program.
- A minimum of one year of experience in ambulatory or hospital nursing is preferred.
- Current RN or LPN license in the State of Michigan.
- BLS Certification to be obtained within two months of employment.
- Strong customer service skills and professionalism.
Primary Responsibilities:
As a Clinic Nurse, you will be responsible for delivering nursing care and supporting healthcare providers in an outpatient setting. Your focus will be on providing ongoing health services to patients, families, and the community. You will:
- Perform advanced nursing functions.
- Collaborate with providers and clinical staff to ensure safe, high-quality patient assessments, treatments, and follow-up care in a patient-centered medical home environment.
- Assess patients, identify abnormalities or urgent issues, and seek appropriate assistance.
- Work with the care team to advise and refer patients and families to community resources, tracking all referrals.
- Conduct patient assessments and care planning, including self-management goal setting.
- Provide patient education on disease management, including dietary and exercise recommendations.
- Oversee medication management, including immunizations and medication refills according to established policies.
- Conduct in-house laboratory testing following InterCare protocols.
- Manage patient triage, including phone and walk-in assessments, and respond to urgent situations.
- Ensure smooth clinic operations and participate in daily team meetings to enhance patient flow.
Professional Development:
Engage in continuous improvement initiatives and training to enhance patient outcomes and support the development of team members.
Communication Skills:
Demonstrate effective written and verbal communication skills, with the ability to convey complex concepts clearly and engage with diverse populations.
Physical Demands:
This role requires the ability to perform tasks in a typical office environment, including standing, walking, sitting, and lifting up to 25 lbs. Good hand-eye coordination and visual acuity are necessary for using office equipment and reading materials.
