Job Summary

We are seeking a skilled and compassionate Clinic Nurse to join our team at InterCare Health Network. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with providers and other clinical staff to deliver safe and effective patient assessment, treatment, and follow-up care in an ambulatory setting.

Key Responsibilities
  • Provide nursing care and assist healthcare providers in an ambulatory care setting.
  • Assess patients and recognize abnormalities or urgent problems, seeking appropriate assistance as needed.
  • Work collaboratively with providers and other care team members to advise and refer patients to community resources and track referrals.
  • Perform patient assessment and care planning, including self-management goal setting.
  • Remain alert to and communicate information regarding potentially high-risk situations or unsafe patient conditions.
  • Provide patient education, including identification of teaching needs related to disease management.
  • Oversee medication management, including administration of immunizations and other medications and medication refills according to policy and procedures.
  • Perform in-house laboratory testing following InterCare policy and procedure.
  • Responsible for patient triage, including phone triage and walk-ins, and respond appropriately to urgent or emergent patient situations.
  • Participate in daily huddle with the clinical support team, FOS, and provider to review the daily schedule and enhance patient flow.
  • Coordinate care among the clinical support team and help identify patient flow problems and take initiative in their solution.
Requirements
  • Bilingual fluency in Spanish and English.
  • Current Registered Nurse (RN) License in the State of Michigan.
  • Minimum of 2 years of OB/GYN nursing experience preferred.
  • Possesses knowledge of nursing practices acquired through graduation from an accredited nursing program with a minimum of 1 year of ambulatory and/or hospital nursing experience preferred.
  • BLS Certification, to be completed within two months of employment.
Working Conditions

Job duties are performed in the typical office environment of the organization, which requires ordinary ambulatory skills sufficient to visit other locations. The environmental factors and/or physical requirements of this position include the ability to stand, walk, stoop, kneel, crouch, frequently sit, and lift up to 25 lbs, as well as good hand-eye coordination, reach with arms and hands, and finger dexterity.
