Your Role:

The Registered Nurse participating in the Transition to Acute Care (TTAC) program is tasked with delivering and overseeing comprehensive nursing care for designated age-specific populations. This role involves utilizing the nursing process—assessing, planning, implementing, and evaluating—to meet the objectives of the nursing department. Nurses in this training program will work under the close guidance of a preceptor for all primary responsibilities, with the goal of transitioning to independent practice upon successful completion of the training.

Your Contributions to the Team:

  • Conduct timely assessments according to Practice Guidelines, considering physiological, psychological, developmental, sociocultural, spiritual, and lifestyle factors, including signs of abuse or neglect. Make informed decisions and interventions based on patients' actual or potential health conditions.
  • Set measurable and achievable short- and long-term goals for patients based on assessments and diagnoses, assisting in the development and execution of personalized care plans. Prioritize follow-up assessments and adjust care plans as necessary, reflecting a commitment to whole-person care.
  • Exhibit the ability to assess and interpret diagnostic data relevant to patient age and condition, including lab results and interdisciplinary team notes, ensuring knowledge and skills are applied to all diagnoses and procedures.
  • Adhere to all regulatory standards and infection prevention protocols, ensuring medication safety and actively engaging in patient education throughout the care process.

Qualifications:

Essential Skills and Experience:

  • Graduate from an accredited nursing program
  • At least six (6) months of nursing experience
  • Current valid State of Florida or multi-state Registered Nurse license
  • Basic Life Support (BLS) certification
