About the Role

United Hospice is seeking a skilled Physician to lead our Hospice and Palliative Care team. As a key member of our interdisciplinary team, you will be responsible for overseeing the medical care and treatment of our Hospice patients, providing direction and guidance to the clinical team, and ensuring compliance with hospice regulations and agency policies and procedures.


Key Responsibilities

  • Participate in the evaluation and recertification of patients' terminal prognosis at indicated intervals.
  • Collaborate with the interdisciplinary team in the development, implementation, and ongoing revision of the physician plan of care.
  • Complete documentation of all patient care services provided.
  • Intervene when there is a possible revocation; review all revocations with the other members of the IDT.
  • Provide medical expertise to admission and patient care staff on issues and challenges involving evaluation of terminal prognosis and pain and symptom management.
  • Collaborate with attending, referring, and/or inpatient physicians regarding issues and challenges involving the determination of terminal prognosis and pain and symptom management, as well as the transfer of patients from home care to inpatient care.
  • Provide home visits based on the agreed-upon plan of care and as directed by the Clinical Director based on patient and family needs.
  • Provide for the general medical needs of patients to the extent that these needs are not met by the attending physician.


Requirements

  • Licensed and currently registered to practice medicine or osteopathy in New York State.
  • DEA license required.
  • Board-eligible/certified in Internal Medicine, Family Practice, or other related specialty with a working knowledge of the principles of palliative medicine and symptom management.
  • A valid driver's license may be required, as determined by operational/regional needs.
  • PECOS enrollment is required.
