Position Overview:
As the inaugural member of the care team for GoGoCare, you will play a pivotal role in establishing best practices, executing key deliverables, and contributing to the ongoing development of the program.
The Client Care Coordinator will be tasked with enrolling and onboarding new clients into the program while providing prescription management and care navigation services via phone.
This position reports directly to the Healthcare Operations Manager and requires the ability to work independently with minimal supervision.This is a full-time remote position available only to candidates based in the United States, requiring a minimum commitment of 40 hours per week during the hours of 8:00am to 5:00pm EST.
The role is classified as non-exempt, with an annual salary range of $54,000-$58,000, based on experience.Key Responsibilities:
Act as the primary contact for GoGoCare services.
Facilitate the enrollment of new clients into GoGoCare.
Deliver telephonic prescription management services, including collaboration with clients' healthcare providers and pharmacists to secure timely medication refills.
Conduct medication assessments and provide recommendations to healthcare providers.
Educate clients about their medications and potential side effects.
Address general inquiries regarding medications.
Evaluate client needs and deliver comprehensive care navigation and coordination services.
Document client interactions and track client outcomes.
Assist in developing program processes and best practices.
Collaborate with various team members, including social workers, engineers, and contact center representatives.
Qualifications:
Valid and active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license. Compact state or California license preferred.
A minimum of 3-5 years of experience working with geriatric populations; management experience is a plus.
Strong expertise in medication management and care navigation.
Experience in conducting clinical assessments over the phone.
Excellent customer service skills, particularly in care coordination and education via phone.
Exceptional verbal and written communication abilities.
Proficient in computer skills, including experience with Electronic Health Records (EHR) and Google Workspace; familiarity with Slack is preferred.
Preferred Skills:
Bilingual in Spanish is advantageous.
