About the Role:

As an Advanced Practice Provider (APP), you will play a vital role in delivering compassionate care to elderly patients with chronic and acute conditions in their homes.

Your responsibilities will include:

  • Providing primary, urgent, and acute medical care to a diverse patient population.
  • Maintaining accurate patient records and scheduling appointments as needed.
  • Collaborating with a multidisciplinary team to ensure comprehensive patient care.

Key Responsibilities:

  • Engage patients in the WellBe program and provide primary care in their homes.
  • Facilitate decision-making for staff, patients, and families by offering educational resources.
  • Conduct initial screenings and physical examinations, developing appropriate care plans.
  • Address urgent care needs and collaborate with the community team for comprehensive assessments.
  • Prescribe and manage medications and treatments as necessary.
  • Maintain accurate documentation and ensure compliance with company metrics.
  • Provide ongoing care information and support to patients with critical conditions.
  • Participate in interdisciplinary team meetings and maintain required licensure.

Qualifications:

Essential Requirements:

  • Master's Degree in Nursing
  • Valid nurse practitioner or physician assistant license and current board certification
  • Minimum of 5 years of progressive care experience
  • BLS and ACLS certification, along with an NPI#
  • Valid driver's license and auto insurance, with the ability to travel to patient locations

Skills and Abilities:

  • Demonstrated teamwork, integrity, and productivity
  • Expertise in diagnosing and treating chronic and urgent conditions in older adults
  • Strong communication skills, both verbal and written
  • Proficient in computer applications, including Word, Excel, and PowerPoint
  • Ability to connect with patients and families effectively
  • Excellent time management and multitasking capabilities

Travel Requirements:

Local travel may be required up to 100%.


Work Conditions:

Ability to lift up to 20 lbs; moving or transferring patients may require lifting up to 50 lbs.
