Licensed Practical Nurse for Hospice Care

A skilled Licensed Practical/Vocational Nurse provides essential nursing care to patients facing terminal illnesses. This role is executed in alignment with physician directives and the Interdisciplinary Group (IDG) care plan, under the oversight of a Registered Nurse. Services are delivered in accordance with established Hospice protocols. The nurse plays a critical role in assessing the patient's physical, psychological, and environmental needs, as documented through clinical records and team evaluations.
KEY RESPONSIBILITIES:
- Adheres to established policies and procedures consistently.
- Implements the nursing care plan as outlined by the IDG for each patient.
- Administers nursing services, treatments, and preventive procedures as designated.
- Monitors and reports any signs, symptoms, and reactions to treatments, including medications, to the physician and Registered Nurse.
- Educates and supports patients and their families regarding nursing care requirements and related issues at home.
- Collaborates with the Registered Nurse to evaluate the effectiveness of nursing services provided to patients and families.
- Keeps precise and comprehensive records of patient observations, treatments, and care.
- Participates in medical record audits as required.
- Attends scheduled staff meetings, IDG conferences, and in-service training.
- Handles on-call duties, including nights, weekends, and holidays as assigned.
- Responsible for:
- Communicating any schedule changes to the Director or Manager of Patient Services daily.
- Engaging in IDG conferences to discuss the involvement of other health team members.
- Prepares clinical and progress documentation.
- Assists the physician and Registered Nurse with specialized procedures.
- Prepares necessary equipment and materials for treatments.
- Maintains aseptic techniques as required.
- Guides patients and families in learning appropriate self-care practices.
LIFTING REQUIREMENTS:
- Ability to engage in physical activities.
- Capacity to stand and be physically active for extended periods.
- Ability to perform extensive bending, lifting, and standing regularly.
QUALIFICATIONS:
- Graduate of a state-approved practical (vocational) nursing program and currently licensed in the practicing state(s).
- Preferred minimum of one (1) year of nursing experience.
- Alignment with the philosophy and objectives of this Hospice.
- Ability to demonstrate initiative and independent judgment.
- Completion of a Hospice training program prior to providing patient care.
