RN Transplant Coordinator Uncertified

We are seeking a highly skilled and compassionate RN Clinical Transplant Coordinator - Uncertified to join our multidisciplinary team at AdventHealth Rocky Mountain. As a key member of our transplant program, you will provide individualized care and support to patients and their families throughout the transplant process.
Key Responsibilities- Conduct pre-transplant interviews with prospective transplant patients and families to assess eligibility and provide education on the transplant process.
- Interpret eligibility criteria for patients, families, physicians, and outside agencies to ensure seamless coordination of care.
- Assess patients' compliance and ability to adhere to a medication regimen and treatment recommendations.
- Serve as a patient advocate, anticipating the needs of providers, staff, patients, and families.
- Collaborate with the multidisciplinary team to identify and meet the physical and emotional needs of patients, ensuring continuity of care through effective communication and collaboration.
- Minimum of two years' experience in acute care or critical care nursing.
- Current Colorado Registered Nurse licensure.
- Current American Heart Association Basic Life Support Health Care Provider (BLS) certification.
