As a Residential Program Supervisor at Opportunity Partners, you will play a vital role in supporting individuals with disabilities to live independently in their homes and participate in their communities. Your primary responsibility will be to assist these individuals in increasing their independence and quality of life.
Key Responsibilities:- Individual Planning: Participate in the individual planning process with each person served to ensure their needs are met.
- Team Collaboration: Collaborate with a person's care team and facilitate team meetings to ensure effective communication and coordination.
- Recreational Activities: Promote hobbies and recreational activities to enhance the quality of life for individuals served.
- Meal Planning and Grocery Shopping: Assist with meal planning, grocery shopping, and cooking to ensure individuals have access to nutritious food.
- Medical Appointments: Schedule and attend medical appointments and provide medication administration as needed.
- Independent Living Skills: Support and develop independent living skills in individuals, such as safety skills, apartment maintenance, finances, and budgeting.
- Staff Supervision: Supervise a staff of Direct Support Professionals to ensure they are providing high-quality care and support.
- Training and Development: Provide training to personnel on person-served health and support needs to ensure individuals can live as independently as possible.
- Staffing Schedule Management: Manage and maintain staffing schedules to ensure individuals' support needs are met.
- Collaboration with Program Manager: Collaborate with the program manager regarding staff training and coaching to ensure a high-performing team.
- Desire to Make a Difference: A genuine desire to make a positive impact in the lives of individuals with disabilities.
- Independence and Teamwork: Ability to work independently and as part of a team to achieve common goals.
- Reliable Transportation: Reliable transportation and a valid driver's license are required.
- Designated Coordinator Qualifications: Meet one of the following criteria: a 4-year degree in Human Services or a related field plus at least one year of experience in the field, or a bachelor's degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or an associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or a diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or a minimum of 50 hours of education and training related to human services and disabilities and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications listed above.
