Licensed Practical Nurse/Licensed Vocational Nurse

JobRialto is seeking a skilled Licensed Practical Nurse/Licensed Vocational Nurse to join our team. As a key member of our healthcare staff, you will provide high-quality patient care and support to our patients and families.
Key Responsibilities- Provide physical care and emotional support to patients and families under the direct supervision of the Nurse Manager and Medical Director.
- Participate in the planning, implementation, and evaluation of nursing care and treatment plans; observe, report, and record pertinent patient information into electronic health records.
- Support and reinforce patient teaching plans, educate patients about proper medication use, and provide instruction on wound care.
- Administer medications within defined parameters and perform nursing procedures within established guidelines.
- Photograph patients with various skin conditions using various photographic procedures, techniques, and equipment.
- Participate in Quality Assurance activities, support peers and colleagues in maintaining established standards of care, and identify patient care needs and utilize nursing resources appropriately.
- Triage patient urgent clinical calls, schedule appointments, and conduct Telehealth appointments.
- Inform patients of pathology, lab results, and plan of care.
- Utilize In-Basket messages to monitor biopsies and ensure timely administration of therapies.
- Provide clinical nursing coverage across Medical Dermatology specialty services.
- Obtain prior authorizations for medications as needed.
- Participate in developing and implementing new ideas in nursing practice and contribute to daily activities that promote patient comfort and safety.
- Appropriately utilize and conserve equipment and supplies, order medical supplies, and maintain inventories.
- Provide input to the Nurse Manager regarding perceived unit needs and participate in unit and service committees.
- Ensure the physical environment is maintained, establish and enforce annual fire and safety regulations, and ensure compliance with Joint Commission, OSHA, and Hospital guidelines.
- Troubleshoot routine and non-routine problems and take measures to correct or handle issues.
- Maintain and update clinical knowledge and skills.
- Participate in relevant continuing education programs.
- Attend Nurses Association meetings.
- Demonstrate knowledge of and dedication to the highest patient care and provide service excellence to all patients.
- Exhibit a can-do, team-oriented attitude, ability to work independently, and interact easily with physicians, managers, and staff.
- Effective interpersonal, communication, organizational, and flexibility to handle multiple tasks.
- Administration of injections.
- Current state licensure as a Licensed Practical Nurse required.
- CPR certification required.
- Phlebotomy experience strongly preferred.
- Graduate of an approved LPN program required.
- LPN experience preferred.
