Job Summary

We're seeking a compassionate and skilled Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our Home Health team. As a Home Health Nurse, you'll provide high-quality patient care in the comfort of their own homes.

Responsibilities

  • Provide nursing visits according to the treatment plan prescribed by the physician
  • Maintain accurate and complete medical records
  • Collaborate with the healthcare team to ensure seamless care
  • Communicate effectively with patients and their families

Requirements

  • One year of clinical experience and one year of home health experience
  • Graduate of an accredited practical or vocational nursing program
  • Currently licensed LPN or LVN in the state of agency operation
  • Valid CPR certification
  • Licensed driver with a valid driver's license
