Job Title: Client Case Manager

Avery's Village is seeking a highly skilled and compassionate Client Case Manager to join our team. As a key member of our staff, you will be responsible for providing exceptional support and care to our youth clients.

Key Responsibilities:
  • Manage a caseload of up to 16 youth clients, providing individualized support and services to meet their unique needs.
  • Collaborate with child placement agencies, therapists, schools, and other outside agencies to ensure seamless care and support for our clients.
  • Conduct initial assessments, create and maintain case books, and develop and implement individualized treatment plans.
  • Coordinate client admissions, ensure timely completion of health examinations, and facilitate medication and therapy appointments.
  • Write weekly and monthly progress reports, create transition plans, and ensure successful client discharge.
  • Provide crisis intervention, transportation, and support for clients as needed.
Requirements:
  • Bachelor's degree in a Human Services-related field; master's degree preferred.
  • 2-5 years of experience as a case manager, with a strong background in mental health theory and practice.
  • Proficient technology skills, including Microsoft Office suite and ability to work independently.
  • Excellent interpersonal and communication skills, with the ability to work effectively with diverse staff and clients.
Desirable Qualifications:
  • Current CPR/First Aid Certification.
  • Knowledge and sensitivity in working with culturally diverse populations.
  • Strong organizational and planning skills, with the ability to prioritize tasks and manage multiple projects.
