Job Title: LPN (Licensed Practical Nurse) - Case Management

We are seeking a highly skilled LPN (Licensed Practical Nurse) to join our team in Kansas City, Kansas. As a case manager, you will be responsible for the care and management of patients with tuberculosis (TB) disease.

Key Responsibilities:

  • Develop and implement individualized case management plans for TB patients
  • Conduct home visits and other assessments to ensure patient compliance with treatment
  • Collaborate with the TB team to promote best patient outcomes
  • Conduct Directly Observed Therapy (DOT) for TB treatment in clinic and home health settings
  • Conduct TB testing, sputum collections, and other specimen collections

Requirements:

  • Associate degree from an accredited school of nursing
  • Valid state licensure as a LPN
  • Valid driver's license
  • Minimum 1-2 years of experience
  • Current BLS certification
