Job Summary

We are seeking a highly motivated and experienced Case Manager to join our team at The Salvation Army Central Territory. As a Case Manager, you will be responsible for coordinating social services activities, providing direct assistance and referrals to clients, and assessing families for eligibility in the Pathway of Hope (POH) program.

Key Responsibilities
  • Conduct outreach and engagement activities to identify eligibility requirements and build rapport with target populations.
  • Perform intake and assessment of potential applicants, including required documentation and screening for serious personal safety and mental health issues.
  • Develop and implement case plans with participants, including short-term and long-term goals and objectives.
  • Provide information and referral services, assist participants in connecting to community resources, and provide advocacy services as needed.
  • Coordinate case management efforts with staff and contracted services to meet individual and family needs.
  • Track and report unmet needs of participants and their families, and document and report effectiveness of service delivery.
Requirements
  • Minimum of two years of case management experience in a comparable social service program.
  • Experience and/or strong interest in community outreach, organization, and community capacity development.
  • Ability to collaborate on complex social issues within families and communities.
  • Ability to think logically and critically, and to solve complex problems and make appropriate judgments and decisions.
  • Excellent oral and written communication skills, and ability to speak and understand English in a manner that is sufficient for effective communication.
