Job Summary

We are seeking a skilled Complex Care Social Worker to join our team at Grace Medical Center. As a key member of our interdisciplinary team, you will play a critical role in facilitating discharge planning for patients with complex needs.

Using a multi-step escalation protocol, you will identify patients with barriers to effective discharge and work collaboratively with the team to develop and implement a plan of care. Your expertise will be essential in monitoring high-cost patients and identifying early intervention opportunities to effect a timely and cost-saving discharge.

As a seasoned professional, you will bring a deep understanding of the complexities of care management and a passion for delivering high-quality patient outcomes. Your ability to communicate effectively with patients, care team members, and community partners will be invaluable in this role.

Responsibilities
  • Develop and implement discharge plans for patients with complex needs
  • Collaborate with the interdisciplinary team to identify and address barriers to discharge
  • Monitor high-cost patients and identify opportunities for early intervention
  • Communicate effectively with patients, care team members, and community partners
  • Provide leadership and creativity for programs and projects to enhance the effectiveness of care management
Requirements
  • Master's degree in social work or related field
  • Current/valid license in the State of Maryland (LCSW-C/LCPC required)
  • Minimum of 3 years of experience in care management or related field
