Job Summary:

WestCare is seeking a skilled Case Manager to join our team in managing a significant caseload of participants in the STOP Program. This role involves case management, documentation, coordination of services, and travel to various locations.

Key Responsibilities:

  • Manage a caseload of participants released from prison substance abuse programs.
  • Coordinate services between multiple agencies, including healthcare providers, social services, and community-based organizations.
  • Travel to community-based providers to ensure seamless service delivery.
  • Develop and implement in-prison presentations to educate participants about the STOP Program.
  • Collaborate with parole services to ensure participant compliance and success.
  • Embody and reflect the mission and goals of WestCare, a leading provider of behavioral health services.
  • Perform other assigned duties as needed.

Requirements:

To succeed in this role, the ideal candidate will possess:

  • Knowledge of the criminal justice system and its complexities.
  • Strong interpersonal skills, with the ability to build rapport with participants, colleagues, and community partners.
  • Organizational abilities, with a focus on attention to detail and effective time management.
  • Computer proficiency, with experience in case management software and other relevant tools.
  • Adherence to ethical standards, with a commitment to confidentiality and participant-centered care.
  • Professionalism and cultural sensitivity, with the ability to work effectively with diverse populations.
  • A valid driver's license, with a clean driving record.
  • Certifications in CPR, First Aid, and clearance tests, as required.

Education and Experience:

  • Two years of experience working with a criminal justice population, preferably in a case management or social services role.
  • Previous case management experience, with a focus on substance abuse treatment and recovery.
  • A college degree in a related field, such as social work, psychology, or counseling.
  • A high school diploma or equivalent, with a strong foundation in communication and interpersonal skills.

Working Conditions:

  • Office, residential, or outpatient setting, with flexibility to work in various locations.
  • Regional travel expected, with occasional overnight stays.
