We are seeking a highly skilled Pediatric Community Resource Specialist to join our team at HealthCare Support. As a Pediatric Community Resource Specialist, you will play a critical role in providing care management services to assist with coordination of care for pediatric patients.
Key Responsibilities- Provide field-based care management services to pediatric patients, including assessing health care needs, referring to health plan providers, and providing education on benefits and community resources.
- Manage a caseload of 100 members, including children in foster care, children who have been adopted from foster care, and young adults in extended foster care.
- Document all interactions and care plans in an electronic health record, ensuring compliance with care management policies and procedures.
- Bachelor's degree in a related field.
- At least 1 year of experience in pediatric case management.
- Proven ability to connect families and children with local resources, including employment, housing, independent living, and justice services.
- Valid and active driver's license, reliable transportation, and insurance.
- Excellent documentation skills and experience with MS Office, including Excel, Outlook, and Word.
- Immediate enrollment in health insurance.
- Dental insurance.

- Career and educational tools within our Ingenovis ACT (Advocacy) Program.
$30 per hour, paid weekly on a W-2 basis.
