We are seeking a skilled Licensed Practical Nurse to join our team at Florida Home Health. As a key member of our home health care team, you will provide high-quality patient care and technical skilled care in the comfort of patients' homes.
Key Responsibilities- Provide Quality Patient Care
- Deliver high-quality clinical services within the scope of practice, as defined by state laws governing nursing practice, in accordance with the plan of care, and in coordination with other members of the healthcare team, following all applicable infection control procedures.
- Continually observe and report patients' nursing needs to supervising RNs.
- Documentation and Care Delivery
- Communicate effectively with physicians, patients, family members, and agency staff regarding patient care issues or needs, visit assignments, and schedule changes.
- Document patient visits thoroughly and completely, adhering to policy and payer requirements, and sync timely with LHC policy.
- Quality and Professionalism
- Actively participate in the performance improvement plan, working to reduce patient hospitalizations and improve outcomes.
- Attend and actively participate in all meetings, including case conferences, and maintain a positive attitude, willing to help patients and the agency in any way possible.
- Teamwork and License Requirements
- Adhere to and participate in the agency's Episode Management process.
- Hold current LPN licensure in the state of practice, a current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
