About the Opportunity:

We are seeking a highly motivated and detail-oriented individual to join our team as a Community Resource Navigator. In this role, you will be responsible for providing exceptional customer service to patients and families, navigating them through the healthcare system, and connecting them with community resources.

Key Responsibilities:

  • Provide patient navigation services to ensure seamless transitions of care
  • Develop and maintain relationships with community partners to enhance access to healthcare resources
  • Conduct home visits as necessary to aid newly assigned and high-risk patients

Requirements:

  • Bachelor's degree in Social Services or Business Administration (or equivalent experience)
  • At least 2 years of customer service experience, preferably in a healthcare environment
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
