Position Overview

The hospice registered nurse employs the nursing process to deliver hospice and palliative care to patients facing terminal illnesses, while also providing essential support to their families within the Hospice House.


Compliance with all organizational policies and procedures is mandatory, including but not limited to: maintaining licensure, completing required training, adhering to employee health protocols, following attendance policies, upholding dress code standards, ensuring patient confidentiality, and practicing infection control and medication administration.

Demonstrates a comprehensive understanding of regulatory agency requirements (The Joint Commission, DHEC) and provides care in accordance with the SC Nursing Practice Act.



Key Responsibilities

Assessment and Planning

a. Conducts thorough assessments of patients' physiological, psychosocial, developmental, and environmental needs, as well as the supportive needs of their families. Takes into account the physical, emotional, and spiritual dimensions of the patient and family during evaluations. Assesses pain and other symptoms, contributing to the interdisciplinary Plan of Care based on findings. Documents assessments accurately in the medical record.

b. Develops personalized nursing and CNA care plans, engaging input from patients and families. Regularly monitors and revises care plans in response to changes in patient status and family needs.

c. Provides education to patients and families, assessing their needs for additional services and referring them to other hospice team members as necessary.

d. Creates a plan of care for the time of death that aligns with the expressed wishes of the patient and family.

Implementation

a. Executes appropriate nursing interventions with meticulous and timely documentation in the patient's chart, utilizing standing orders and medical guidelines to inform practice.

b. Communicates assessment findings and any changes in the patient's condition to the physician, making recommendations as needed.

c. Acknowledges the family's role in decision-making and the patient's right to self-determination.

d. Supervises the performance of CNAs in accordance with the established Plan of Care.

Evaluation

Assesses the effectiveness of specific interventions and the progress towards established goals. The evaluation process reflects the patient's response to treatments and medications, disease progression, family responses to changes, and the overall effectiveness of teaching provided to the patient and family.

Bereavement Support

Recognizes the significance of ongoing support for families during bereavement, providing follow-up services, attending memorial events, and participating in scheduled bereavement programs. Demonstrates knowledge of grief and loss processes and offers appropriate supportive interventions.

Continuity of Care

a. Ensures continuity of care through effective communication with team members and external agencies, facilitating timely information exchange.

b. Engages in regular communication with all staff and volunteers regarding patient status and collaborates with physicians and others to ensure quality care.

c. Accurately documents all clinical findings, interventions, physician orders, changes in care plans, and communications with team members.

d. Initiates the Plan of Care at the time of death and notifies all relevant parties. Completes all necessary discharge documentation.

Agency Contributions

Supports departmental needs by:

a. Participating in quality data reviews and making recommendations for improvements.

b. Conducting admission and evaluation visits as required.

c. Engaging in various projects and committees as requested.

d. Educating the community on end-of-life issues.

e. Assisting in the orientation and training of new employees and volunteers.

Supervisory Responsibilities

This position does not include management responsibilities and reports to a supervisor, manager, director, or executive.

Education Requirements

Associate's Degree in Nursing is required.

Experience Requirements

No prior experience is necessary; however, 2 years of acute care nursing experience is preferred.

Certifications and Licenses

Registered Nurse (RN) license is required.

Must possess a valid driver's license, an acceptable motor vehicle record, and proof of auto insurance.

Preferred Qualifications

Bachelor of Science in Nursing (BSN) is preferred.

Experience in hospice or home care services is also preferred.

Work Schedule
Night shifts are available.
