The Hospice Registered Nurse is a vital member of our interdisciplinary team, providing compassionate and high-quality care to patients and their families in our Hospice House. As a Nurse Care Specialist, you will utilize the nursing process to assess, plan, implement, and evaluate patient care, ensuring that each individual receives the best possible support and services.

Key Responsibilities
  • Assessment and Planning
    • Conduct thorough assessments of patients' physiological, psychosocial, and environmental needs, as well as the needs of their families.
    • Develop individualized nursing and CNA plans of care, seeking input from patients and families as needed.
    • Monitor and update plans of care in response to changes in patient status and family needs.
  • Implementation
    • Perform nursing interventions with thorough and timely documentation in patients' charts.
    • Utilize standing orders and medical authorization guidelines to guide practice.
    • Report assessment findings and changes in patient condition to physicians and make recommendations as needed.
  • Evaluation
    • Evaluate the effectiveness of specific interventions and the resulting movement toward goals.
    • Assess patient response to treatment and medication, disease progression, and family response to change.
  • Bereavement Support
    • Provide ongoing support to families during the bereavement process, including bereavement follow-up, attending funeral services, and participating in bereavement program events.
  • Continuity of Care
    • Facilitate continuity of care with other team members and agencies through accurate and timely exchange of information.
    • Communicate regularly with staff and volunteers regarding patient status.
  • Agency Needs
    • Participate in review of quality data and make recommendations for improvement.
    • Perform admission, evaluation visits, and consultative services as requested.
    Requirements
    • Associate's Degree in Nursing
    • Valid RN license
    • 2 years of acute care nursing experience preferred
    • BSN preferred
    • Experience in hospice or other home care services preferred
