Job Overview

The hospice registered nurse employs the nursing process to deliver hospice and palliative care to patients facing terminal illnesses, irrespective of age, while also providing essential support to their families within the Hospice House.

Compliance and Standards

Upholds all compliance, policies, and procedural requirements of the organization, including:
- Licensure renewal
- Mandatory training
- Employee health screenings
- Adherence to time and attendance policies
- Dress code compliance
- Patient confidentiality
- Infection control protocols
- Medication administration guidelines

Demonstrates a comprehensive understanding of regulatory agency standards, including The Joint Commission and DHEC, while providing care in accordance with the SC Nursing Practice Act.

Key Responsibilities

Assessment and Planning
a. Evaluates the physiological, psychosocial, developmental, and environmental needs of patients and their families, considering their emotional and spiritual requirements. Assesses pain and other symptoms, contributing to the interdisciplinary Plan of Care based on these evaluations. Documents findings in the medical record.
b. Formulates personalized nursing and CNA care plans, incorporating input from patients and families, and revises them as necessary in response to changes in condition and family needs.
c. Provides education to patients and families, monitoring their need for additional services and coordinating with other hospice team members.
d. Develops a care plan for end-of-life that honors the expressed wishes of the patient and family.

Implementation
a. Executes appropriate nursing interventions with thorough and timely documentation in the patient's chart, utilizing standing orders and medical guidelines to inform practice.
b. Communicates assessment findings and changes in patient condition to the physician, making recommendations as needed.
c. Acknowledges the family's role in decision-making and the patient's right to self-determination.
d. Supervises the performance of CNAs in accordance with the Plan of Care.

Evaluation
Assesses the effectiveness of interventions and progress toward goals, considering patient responses to treatments, disease progression, and family dynamics.

Bereavement Support
Recognizes the significance of ongoing support for families during bereavement, engaging in follow-up, attending memorial services, and participating in bereavement programs.

Continuity of Care
a. Ensures seamless communication and information exchange with team members and external agencies to facilitate continuity of care.
b. Regularly updates all staff and volunteers on patient status and collaborates with physicians to maintain quality care.
c. Accurately documents all clinical findings, interventions, and changes in the Plan of Care.

Agency Support
Contributes to departmental needs by:
- Participating in quality data reviews and making recommendations for improvements.
- Conducting admission evaluations and consultative services as required.
- Engaging in various projects and committees.
- Educating the public on end-of-life issues.
- Assisting in the orientation and training of new staff and volunteers.

Supervisory Responsibilities

This position does not include management responsibilities and reports to a supervisor, manager, director, or executive.

Qualifications

Education: Associate's Degree in Nursing
Experience: No prior experience required; 2 years of acute care nursing preferred.
Certifications: RN - Registered Nurse (Code NLRN)
Must possess a valid driver's license and maintain an acceptable motor vehicle record.

Preferred Qualifications:
- BSN
- Experience in hospice or home care services

Work Schedule

Night shifts available.
