Position Overview

The hospice registered nurse employs the nursing process to deliver hospice and palliative care to patients with terminal illnesses, irrespective of age, while also providing essential support to the patient's family within the Hospice House.


Complies with all organizational policies and procedures, including but not limited to: licensure renewal, mandatory training, employee health assessments, attendance protocols, dress code adherence, patient confidentiality, and infection control measures. Demonstrates understanding of regulatory agency standards (The Joint Commission, DHEC) and provides care in accordance with the SC Nursing Practice Act.

Key Responsibilities

Assessment and Planning

a. Evaluates the physiological, psychosocial, developmental, and environmental needs of the patient and their family, taking into account the physical, emotional, and spiritual dimensions of care. Assesses pain and other symptoms, contributing to the interdisciplinary Plan of Care based on evaluations. Documents assessments in the medical record.

b. Formulates a personalized nursing and CNA care plan, incorporating feedback from the patient and family. Continuously monitors and adjusts care plans as necessary based on changes in patient status and family requirements.

c. Identifies educational needs for the patient/family and provides necessary information. Monitors the need for additional services and coordinates referrals to other hospice team members.

d. Creates a care plan for the time of death that aligns with the expressed wishes of the patient and family.

Implementation

a. Executes appropriate nursing interventions with comprehensive and timely documentation in the patient's chart. Utilizes standing orders and medical guidelines to inform practice.

b. Communicates assessment findings and any changes in the patient's condition to the physician, offering recommendations as needed.

c. Acknowledges the family's role in decision-making and the patient's right to self-determination.

d. Supervises the performance of the CNA in accordance with the Plan of Care.

Evaluation

Assesses the effectiveness of specific interventions and the progress towards established goals. The evaluation process reflects the patient's response to treatments and medications, disease progression, family reactions to changes, and the patient's advancement towards goals and outcomes achieved.

Evaluates the family's capacity to manage the physical and emotional demands of caregiving, considering cultural and developmental needs.


Bereavement Support


Recognizes the significance of ongoing support for families during bereavement, including follow-up with the patient's family, supporting the bereavement Plan of Care, and attending memorial services when feasible. Demonstrates knowledge of grief/loss processes and provides appropriate supportive interventions.


Continuity of Care

a. Ensures continuity of care with team members and external agencies through accurate and timely information exchange. Maintains regular communication with all staff/volunteers regarding patient status. Collaborates with physicians and others as necessary to coordinate and guarantee quality care. Participates in team meetings and related activities.

b. Communicates effectively in writing and verbally with pharmacy, DME vendors, material management, and insurance companies to assist in controlling agency costs.

c. Records all clinical findings, interventions, physician orders, modifications to the Plan of Care, telephone communications, team meetings, and referrals.

d. Initiates the Plan of Care at the time of death and notifies all relevant parties/agencies. Completes all necessary discharge documentation.

Agency Support

Performs various tasks to support departmental needs:

a. Engages in quality data reviews and makes recommendations for enhancements.

b. Conducts admission and evaluation visits and provides consultative services as required.

c. Participates in various projects and committees as requested.

d. Educates the community on end-of-life issues.

e. Assists in the orientation and training of students, new employees, and volunteers.

Supervisory/Management Responsibilities

This position does not involve management responsibilities and reports to a supervisor, manager, director, or executive.

Minimum Education

Associate's Degree in Nursing

Minimum Experience

No prior experience required.

Preferred: 2 years in Acute Care Nursing.

Required Certifications/Registrations/Licenses

RN - REGISTERED NURSE - (Code NLRN)


Must possess a valid driver's license, an acceptable motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto insurance.
