Job Overview

About the Position
The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work collaboratively with our interdisciplinary team. Key Responsibilities
- Provide direct nursing care to residents, including administering medications and performing treatments as ordered by physicians
- Develop and implement individualized care plans that meet the unique needs and preferences of each resident
- Collaborate with the healthcare team to ensure seamless transitions of care and effective communication
- Educate residents and families on disease management and prevention strategies

Requirements and Qualifications
- Active LPN/RN license in the state of employment
- Minimum 1-2 years of experience in gerontological nursing or a related field
- Strong understanding of nursing principles, practices, and standards of care
- Excellent communication and interpersonal skills
