Key Responsibilities:As a Clinical Pharmacist, you will be responsible for accurately filling orders for parenteral and enteral medications, overseeing patient medication therapies, and providing essential drug information. You will also supervise and guide support staff.
In the absence of the pharmacy manager or supervisor, you may be required to undertake critical responsibilities and perform managerial duties.
Specific Duties Include:
- Ensuring precision in the dispensing of all medications.
- Compounding and dispensing pharmaceuticals, including intravenous admixtures, chemotherapy, and parenteral nutrition products with accuracy.
- Issuing controlled substances to patient care areas while maintaining legal records.
- Monitoring drug therapy regimens for contraindications, interactions, allergies, and the appropriateness of medications and dosages.
- Reading, extracting, and interpreting patient chart information accurately.
- Identifying and reporting suspected adverse drug events promptly and accurately.
- Maintaining the formulary by minimizing non-formulary procurements and promoting rational drug therapy choices.
- Providing clinical consultation and clarifications to healthcare practitioners, suggesting appropriate and cost-effective therapeutic alternatives as necessary.
- Delivering accurate drug information to both professional staff and patients/families when required.
- Documenting all clinical activities and interventions thoroughly and accurately.
- Participating in quality improvement initiatives and medication use reviews within the department.
- Engaging in the orientation and training of new employees as directed.
- Working independently with minimal supervision, organizing and prioritizing work assignments to ensure timely pharmacy services.
- Completing and documenting monthly inspections of all assigned medication storage areas comprehensively and punctually.
Applicants must be graduates of an ACPE-accredited School of Pharmacy with a BS in Pharmacy, an MS in Pharmacy, or a Doctor of Pharmacy. A New York State Pharmacy license is required, with oncology pharmacy board certification preferred (BCOP).
Candidates should possess basic and advanced computer skills, particularly in Microsoft Word, Excel, and PowerPoint, and must be able to maintain focus and accuracy in a fast-paced environment.
Exceptional oral and written communication skills are essential, along with the ability to collaborate effectively within a multidisciplinary team. Health system pharmacy experience is preferred but not mandatory.
Compensation:The estimated pay range is USD $79.62 per hour. This range serves as a good faith estimate, and actual compensation will depend on various factors, including qualifications, skills, competencies, and experience.
