About the Job:

We are seeking a compassionate and experienced Registered Nurse to join our team as a Hospice Nurse Care Coordinator. As a member of our interdisciplinary team, you will play a vital role in providing high-quality care to patients and their families.

Job Responsibilities:
  • Conduct comprehensive assessments of patients and develop individualized care plans to meet their unique needs.
  • Provide education and support to patients and their families on managing symptoms, medications, and other aspects of hospice care.
  • Collaborate with the healthcare team to ensure seamless coordination of care and communication with patients and families.
  • Assist with pain and symptom management, as well as other aspects of patient care.
  • Maintain accurate and detailed records of patient interactions, including progress notes and discharge summaries.
Requirements:
  • Current licensure as a Registered Nurse (RN) in the state of employment.
  • At least two years of experience in hospice or palliative care, preferably in a case management role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Basic computer skills and proficiency in electronic medical records (EMRs).
Compensation and Benefits:

Our competitive salary for this position is $85,000 - $95,000 per year, depending on experience. We also offer a comprehensive benefits package, including health insurance, paid time off, and professional development opportunities.
