Job Summary:

Eden Health is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality care to our patients in the comfort of their own homes. As a Hospice LPN, you will play a vital role in delivering patient-centered care, working closely with our interdisciplinary team to ensure the best possible outcomes for our patients and their families.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, as ordered by the physician and in accordance with the plan of care.
  • Collaborate with the supervising RN to review caseload and ensure timely and effective care.
  • Obtain necessary physician orders and work closely with the physician to address patient needs.
  • Assess and educate patients and their families on relevant diagnosis and care plans.
  • Participate in the coordination of hospice services, reporting identified needs to the interdisciplinary team.
  • Use equipment and supplies effectively and efficiently, ensuring patient safety and comfort.
  • Provide pain and symptom management, evaluating patient response to treatments and medications.
  • Actively participate in our Quality Management program, ensuring accurate and thorough visit notes via Electronic Health Record (EHR).
Requirements:
  • Graduate of an accredited nursing program.
  • Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of practice.
  • Experience as an LPN/LVN, preferably in an acute care setting or home care environment.
  • Valid driver's license and reliable transportation.
  • Proof of automobile insurance and a clean driving record.
  • Current CPR certification.
  • Able to pass a criminal background check and national sex offender clearance.
