About the Role:

Key Responsibilities:
  • Assess patient needs and develop personalized care plans
  • Collaborate with healthcare teams to ensure seamless care delivery
  • Maintain accurate and up-to-date patient records
  • Communicate effectively with patients, families, and caregivers

Requirements:
  • Licensed as a Registered Nurse in North Carolina (or eligible for licensure)
  • Minimum one year of experience in acute care or home health/hospice settings
  • Current CPR certification
