Palliative Care Nurse Practitioner Opportunity at Liberty Hospice

We are seeking an experienced Palliative Care Nurse Practitioner to join our team at Liberty Hospice. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and families facing terminal illness.

About the Role:

As a Palliative Care Nurse Practitioner, you will be responsible for implementing palliative care program requirements, conducting face-to-face encounters with patients, and communicating with primary care physicians and supervising medical directors.

Key Responsibilities:

  • Implement palliative care program requirements and ensure seamless coordination of care with the Palliative Care team
  • Conduct regular face-to-face encounters with patients to assess their needs and provide re-certification information to the Medical Director/Associate Medical Director
  • Communicate effectively with patients, families, and healthcare providers to ensure comprehensive care
  • Collaborate with other agency personnel to achieve optimal patient outcomes

Requirements:

  • Master's degree in Nursing required, with current licensure in the state of practice
  • Certification as an Adult or Family Nurse Practitioner
  • Experience in Hospice or Palliative Care nursing preferred, but not required
  • Strong communication and organizational skills, with ability to work effectively in a fast-paced environment
