Licensed Practical Nurse for Personal Care Services

Job Overview:
Status: FLEX/PRN
Shift: All Shifts
Compensation: $31.50/hr plus Shift Differentials including $2 for Evenings, $1.50 for Nights, and an additional $1 for Weekends.

- Ensure seamless communication of essential information during shift changes to maintain continuity of resident care.
- Conduct daily rounds to monitor residents, promptly reporting any issues to the nursing supervisor and taking necessary corrective actions.
- Identify and address safety hazards immediately.
- Supervise Nursing Assistants in providing direct care to residents.
- Administer and accurately document resident care, medications, and treatments as per physician orders.
- Maintain thorough documentation of resident progress notes as required.
- Provide daily guidance to Nursing Assistants regarding care related to treatments.
- Valid and current LPN license in Pennsylvania.
- Strong organizational skills and ability to manage sequential tasks.
- Demonstrated initiative, sound judgment, reliability, and leadership capabilities.
- Exceptional customer service skills with the ability to interact positively with residents, families, physicians, and visitors.
