LSS of WI & Upper MI is seeking a highly skilled Service Coordinator to join our team. As a Service Coordinator, you will play a vital role in supporting our tenants in the Tomahawk, WI area by connecting them with community services and resources.

Key Responsibilities:

  • Provide information and referrals to community connections for tenants in need of supportive services.
  • Assess health, psychological, and social needs of tenants and develop individually tailored case plans.
  • Establish linkages with community agencies and service providers to ensure access to necessary services.
  • Refer and link tenants to service providers, including case management, personal assistance, homemaker services, and more.
  • Monitor the ongoing provision of services and maintain accurate records of tenant progress.
  • Develop and implement volunteer support programs with community organizations.
  • Provide training and education to tenants on tenant rights and responsibilities.
  • Presents workshops and informative sessions to tenants on topics of interest.

Requirements:

  • Bachelor's degree in social work, sociology, psychology, or a related field.
  • At least 3 years of experience working with older adults and/or persons with disabilities.
  • Valid driver's license and reliable transportation.
  • Ability to communicate effectively in verbal and written format.
  • Working knowledge of computers and software applications.
