Field-Based RN Case Manager for Labette, Cherokee, and Crawford Counties

Job Summary

UnitedHealthcare is seeking a highly skilled and compassionate RN Case Manager to join our Field-Based team in Labette, Cherokee, and Crawford Counties. As a RN Case Manager, you will play a vital role in assessing, planning, and implementing care strategies for Medicare and Medicaid members, while proactively engaging them to manage their own health and healthcare.

Key Responsibilities
  • Assess and plan individualized care strategies for members with complex medical and behavioral needs
  • Utilize strong communication and interpersonal skills to engage members and their families in the care planning process
  • Collaborate with interdisciplinary teams to ensure coordinated care and services
  • Manage transitions of care for members discharged from in-patient hospital stays and emergency rooms
  • Develop and update care plans to achieve member goals and improve health outcomes
  • Identify and initiate referrals for social service programs and community resources
  • Communicate with stakeholders to ensure quality coordinated care and services
  • Advocate for members to ensure access to care
Requirements
  • Current, unrestricted RN license in the state of Kansas
  • 2+ years of experience working in the community as a RN in acute inpatient care, long-term care, home health, hospice, public health, behavioral health, or assisted living
  • Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook
  • Ability to travel up to 75% of the time with reliable transportation and a current, non-restricted state of Kansas Driver's License and State-required insurance
  • Access to a designated quiet workspace in your home with the ability to secure Protected Health Information (PHI)
  • Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
Preferred Qualifications
  • Bachelor's degree in nursing
  • A background in managing Medicaid and Medicare populations with complex medical and behavioral needs
  • Experience with arranging community resources
  • Experience with electronic charting
