Healthcare Professional

A healthcare professional is needed to join the Assertive Community Treatment (ACT) team in Queens, New York City, as part of a health services network.
Responsibilities:
The ideal candidate will be responsible for:
Conducting mental status exams to assess patients’ mental health
Assessing physical health needs and making referrals to Community Physicians as necessary
Providing management and administration of medications in conjunction with Psychiatrists
Participating in a team that delivers an integrated array of treatment, rehabilitation, and support services, customized to each individual’s needs
Providing the majority of services in the community, with participation in rotating 24-hour crisis coverage and response
Performing other duties as needed
Qualifications:
1+ year of experience in a Rehabilitation and/or Treatment setting
Graduate of an accredited School of Nursing
NYS Registered Nurse (RN) license
Desired Skills:
Bachelor’s Degree in Nursing
Knowledge of Nursing Assessment methods
