- Facilitate the needs and aspirations of each individual by accompanying them to medical appointments, visits with family and friends, preferred leisure activities, and encouraging social interactions with peers.
- Adhere to all physician and nursing staff directives, including the supervision of self-administration of medication, unless specifically trained as a Proxy.
- Organize annual medical and dental appointments as required by nursing staff or the Individual Service Plan (ISP).
- Execute approved emergency protocols in the event of a fire, storm, or other disasters, whether natural or manmade.
- Engage in the formulation of ISP goals.
- Deliver training to individuals as specified in the ISP to help them achieve their objectives.
- Maintain daily documentation in compliance with DBHDD standards and any other daily data collection as outlined in the Agency's program design.
- Keep records of medical and dental visits, current weight, medication details (dosage, frequency, times of administration, reasons for medications, side effects, illnesses, injuries, special incidents, personal funds, and possessions).
- Ensure meals are prepared according to established menus.
- Uphold house rules consistently.
- Maintain a clean, safe, and healthy living environment by performing household cleaning, notifying the House Manager of any broken or inoperable items, securing medication and hazardous materials, and reporting any potential safety issues.
- Report any unusual occurrences or health and safety concerns to the House Manager promptly.
- Notify the House Manager of any suspected individual abuse as soon as it is discovered or reported by the individual or other concerned parties.
