About the Role

KurzSolutions is seeking a skilled Registered Nurse to join our team as a Women's Health Care Coordinator. In this role, you will provide professional nursing services in case management for patients, including adult and pediatric case management within Ambulatory Care and/or Women and Children's services.

Responsibilities
  • Apply knowledge of the nursing process to deliver individualized, goal-oriented care.
  • Develop, implement, and evaluate patient care coordination and case management programs to adhere to current standards of care.
  • Collaborate with primary care providers, clinic nurses, public health nursing, case management, and social services to meet patient care needs efficiently and cost-effectively.
  • Manage clinic and follow-up visits, coordinating with multidisciplinary teams including physicians, nursing personnel, social workers, and physical therapists.
  • Provide information to providers and assist in implementing treatment plans according to age-appropriate health maintenance needs.
  • Administer prescribed oral medications, intramuscular injections, and intravenous medications as ordered by physicians.
  • Coordinate and maintain effective communication channels with other departments, nursing units, and tribal/community agencies.
Requirements
  • Must have a minimum of an Associate of Science in Nursing degree; Bachelor's degree is preferred.
  • Shall possess a current, valid, unrestricted nursing license in a state, the District of Columbia, the Commonwealth of Puerto Rico, or a U.S. Territory.
  • Must have a minimum of 36 months of nursing experience, with at least 2 years in women's health.
  • It is mandatory that the applicant possesses current Basic Life Support (BLS), Pediatric Advanced Life Support (PALS) and Advanced Cardiac Life Support (ACLS) certifications.
