Program Coordinator Job Description

At Constructive Partnerships Unlimited, we are seeking a highly skilled and dedicated Program Coordinator to join our team in the Bronx Community Living Program. As a Program Coordinator, you will play a vital role in developing, implementing, and monitoring program plans for individuals with intellectual and developmental disabilities.

Key Responsibilities:

  • Develop and implement program plans that promote individualization, independence, inclusion, and productivity.
  • Provide training to direct care staff on program implementation and supervise staff in the area of programming and data collection.
  • Chair and participate in annual, semi-annual, and special team conferences for individuals receiving services.
  • Complete required documentation, including minutes, program plans, and annual updates.
  • Ensure monthly notes are completed correctly and records are maintained and documentation is uploaded accurately.
  • Integrate services with Day Programs and ensure clinical documentation is completed in compliance with Agency Policy and Procedures.
  • Ensure Habilitation Billing forms are documented, reviewed, submitted, and uploaded as regulated for IRA residences.
  • Attend staff cluster meetings and communicate with management daily to ensure service plans are being implemented.
  • Schedule and conduct special team meetings as needed, in conjunction with Care Managers.

Requirements:

  • Bachelor's degree in a related human services field.
  • Minimum of two years supervisory experience.
  • QIDP status and knowledge of HCBS Waiver and IRA/ICF regulations.
  • Minimum two years program experience working with individuals with intellectual or developmental disabilities.
  • Ability to provide weekend coverage as scheduled and work flexible hours as needed.
  • Working knowledge of Microsoft Office Applications.
