Position Summary
The role of Nursing Program Coordinator at RiverSpring Health, a reputable long-term care provider, involves delivering community leadership and nursing services to clients and their families. The Nursing Program Coordinator is tasked with providing healthcare services to clients in the Assisted Living Residence (ALR) in accordance with the organization's guidelines and under the oversight of a Registered Professional Nurse (Director of Nursing).
Key Responsibilities
  • Assisting in the development, execution, and continuous evaluation of the nursing care strategy.
  • Supervising the care teams and managing daily operations.
  • Monitoring, documenting, and communicating changes in clients' health status, reactions, and symptoms.
  • Carrying out specific nursing tasks within the scope of practice, including the administration of medications and treatments.
  • Overseeing medication management for clients, including updates, new prescriptions, and discontinuations.
  • Facilitating admissions, conducting team meetings with care staff, performing daily reviews of new and existing client records, and managing incident reports and follow-ups.
Qualifications
A graduate from an accredited practical nursing institution with a valid New York State License and current registration as a licensed practical nurse as per Article 139 of the New York State Education Law. A minimum of one to two years of experience as a licensed practical nurse in community health, home care, acute care, or adult care settings is required. Experience in supervision and medication management is essential.
Compensation: $40 per hour, Monday - Friday with rotating weekends.
