RN - PACE/Elderplace Care Specialist

We are seeking a skilled and compassionate RN to join our PACE/Elderplace team as a Community Care RN. In this role, you will provide and supervise professional, independent nursing care for our patients, promoting their optimal health and independence.
Key Responsibilities- Assess, plan, and implement individualized care plans for patients
- Supervise and delegate nursing care to other healthcare professionals
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Provide education and support to patients and their families
- Participate in quality improvement initiatives to enhance patient outcomes
- Current Oregon Registered Nurse License
- National Provider BLS - American Heart Association certification
- Relevant certifications applicable to the position, as specified by the Nurse Manager
- Bachelor's Degree in Nursing (preferred)
- 1 year of Home health RN experience (preferred)
