Responsibilities
  • Collaborate with healthcare providers to develop and implement personalized care plans
  • Conduct regular follow-ups with patients to monitor progress and adjust care plans as needed
  • Communicate effectively with patients, families, and healthcare teams to ensure smooth transitions
  • Maintain accurate records and reports to track patient outcomes and program effectiveness
Qualifications
  • Degree in RMA or Nursing (RN or LPN)
  • 2+ years of experience in direct patient care or managed care
  • Strong analytical and problem-solving skills
  • Excellent communication and interpersonal skills
