About the Role

At EMrecruits, we are committed to providing high-quality care to our patients. We are currently seeking a Transition of Care Specialist to join our team. This is an exciting opportunity for a dedicated and organized individual to make a positive impact in the lives of our patients.

The ideal candidate will have experience in transitional care services, including medication reconciliation and disease management. They will be responsible for working closely with our primary care providers and community agencies to ensure seamless continuity of care.

Key Responsibilities:

  • Develop and implement individualized care plans in collaboration with primary care providers
  • Provide education to patients on the importance of medical care management and how to access care within the medical home environment
  • Assist patients in addressing concerns through referral for assessment, counseling, and communication with healthcare teams
  • Maintain accurate client documentation in electronic health records

Essential Requirements:

  • Degree in RMA or Nursing (RN or LPN)
  • Experience in transitional care services (highly preferred)
  • Valid driver's license and personal vehicle for travel between work sites
  • HEDIS Quality Measures experience
  • Excellent communication and customer service skills
  • Ability to work independently and collaborate with other team members

Salary and Benefits: We offer a competitive salary range of $65,000 - $90,000 per year, depending on experience. Additionally, we provide a comprehensive benefits package, including health insurance, retirement plan, and paid time off.
