ESSENTIAL JOB FUNCTIONS:
1. Conduct regular meetings with residents and their families. Collaborate with family members to foster constructive relationships and resolve conflicts.
RESPONSIBILITIES:
2. Perform a comprehensive social assessment and create an Individualized Service Plan (ISP) for each resident.
- Support residents in adapting to their new environment.
- Establish formal and informal communication channels with all staff members involved in resident care.
- Provide assistance and support to residents and their families during transitions to alternative care settings.
- Contribute to the activity program with insights and support.
- Engage in community initiatives related to mental health.
- Offer counseling services to residents facing crises.
- Identify residents needing counseling for issues such as substance abuse or depression.
- Collaborate with residents to uncover their unmet needs.
- Work alongside Administration to establish adequate aftercare programs tailored to residents' needs.
- Coordinate resident services with external resources, including professionals and volunteers.
- Monitor and report any changes in residents' mental or physical health to the appropriate authorities.
- Create initial case records upon resident admission and update them as necessary to ensure continuity of care.
- Assist residents in accessing income maintenance, health, and social services.
- Compile a resource directory of services, benefits, and entitlements available to residents.
- Visit residents during hospital stays.
- Maintain accurate records of residents' benefits and ensure medical benefits are current.
- Facilitate recognition of important dates such as birthdays and anniversaries.
- Support the resident council.
- Represent the facility in community engagements as needed.
- Review residents' service needs periodically and advocate on their behalf, addressing any complaints that arise.
- Ensure all residents have updated Medical and Mental Health Forms completed annually or when medical conditions change.
- Perform additional responsibilities as assigned by the supervisor.

- Experience and passion for working with older adults.
