POSITION TITLE: Associate Case Manager

SUMMARY: Assist residents in achieving and sustaining optimal social, mental, spiritual, and emotional health. Act as a vital resource for residents and their families.

ESSENTIAL JOB FUNCTIONS:
1. Conduct regular meetings with residents and their families. Collaborate with family members to foster constructive relationships and resolve conflicts.

RESPONSIBILITIES:
2. Perform a comprehensive social assessment and create an Individualized Service Plan (ISP) for each resident.

  • Support residents in adapting to their new environment.
  • Establish formal and informal communication channels with all staff members involved in resident care.
  • Provide assistance and support to residents and their families during transitions to alternative care settings.

  • Contribute to the activity program with insights and support.
  • Engage in community initiatives related to mental health.
  • Offer counseling services to residents facing crises.

  • Identify residents needing counseling for issues such as substance abuse or depression.
  • Collaborate with residents to uncover their unmet needs.
  • Work alongside Administration to establish adequate aftercare programs tailored to residents' needs.
  • Coordinate resident services with external resources, including professionals and volunteers.
  • Monitor and report any changes in residents' mental or physical health to the appropriate authorities.
  • Create initial case records upon resident admission and update them as necessary to ensure continuity of care.
  • Assist residents in accessing income maintenance, health, and social services.
  • Compile a resource directory of services, benefits, and entitlements available to residents.
  • Visit residents during hospital stays.
  • Maintain accurate records of residents' benefits and ensure medical benefits are current.
  • Facilitate recognition of important dates such as birthdays and anniversaries.
  • Support the resident council.
  • Represent the facility in community engagements as needed.
  • Review residents' service needs periodically and advocate on their behalf, addressing any complaints that arise.
  • Ensure all residents have updated Medical and Mental Health Forms completed annually or when medical conditions change.
  • Perform additional responsibilities as assigned by the supervisor.
