Job Summary

We are seeking a highly skilled Patient Advocate Liaison to join our team at Conifer Health Solutions. As a key member of our healthcare team, you will play a vital role in ensuring that patients receive the best possible care and support.

Key Responsibilities
  • Conduct thorough interviews with patients and/or family members to assess their eligibility for various government and non-government programs.
  • Identify and pursue all potential sources of payment, including auto insurance, Workers' Compensation, commercial insurance, private insurance, and more.
  • Obtain and complete confidential financial statements, and assist patients in applying for benefits for which they may be eligible.
  • Develop and maintain strong relationships with patients, based on excellent communication skills, to ensure timely and efficient processing of accounts.
  • Conduct field visits to patient homes for skip tracing and document assistance, as needed.
  • Communicate effectively with hospital case management, social services, and admissions staff to ensure seamless coordination of care.
Requirements
  • High school diploma or equivalent required; minimum 2 years of experience in social services or hospital admitting/related area preferred.
  • Working knowledge of federal, state, and county programs, as well as proficiency in Microsoft Office and Windows.
  • Excellent oral and written communication skills, with a clear understanding of the English language.
  • Ability to work independently, prioritize tasks, and manage multiple responsibilities with efficiency.
  • Bilingualism (Spanish) a plus.
Work Environment

This role requires the ability to sit and work at a computer terminal for extended periods, as well as the ability to walk through hospital environments and visit patients at bedside. Some travel may be required.
