- 2+ years of clinical outpatient experience, post residency
- Current, unrestricted State license or eligibility for licensure

- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
- Active registration with the Drug Enforcement Agency (DEA)
- Active Medicare/Medicaid enrollment or eligibility to enroll
