- Oversee tasks that optimize workflow, identify new protocol opportunities, and provide suggestions to continually improve processes and the patient experience.
- Provide feedback to and work collaboratively with the healthcare leadership team to improve healthcare services, offer additional healthcare services, and achieve healthcare goals.
- Train staff members on workflows and processes.
- Work across disciplines and department teams and community-based organizations to help CommUnityCare meet its organizational goals.
- Train staff in performance of job duties, assign job tasks, and monitor task completion.
- Ensure that all staff are trained to effectively perform their required responsibilities and provided with an overview of the healthcare program performance goals and objectives through regular communication.
- Provide team members with additional training as needed.
- Effectively organize training materials for new employees.
- Oversee the daily activities of the staff to ensure the team meets department standards.
- Answer healthcare team members' day-to-day questions.
- Lead team huddles.
- Create and adjust the healthcare team's monthly schedules.
- Perform grant-related duties per the terms and conditions of the grant, including maintaining all data and programmatic report outs.
- Attend seminars and maintain all licensure and/or certification requirements for continuing education and best practices.
- Develop engaging and informative training material for healthcare, tailored for both CommUnityCare staff and patient audiences.
- Lead dynamic training sessions and presentations.
- Seek opportunities and implement goals for own professional development.
- Develop and maintain favorable internal relationships, partnerships with colleagues, including clinical managers, clinical supervisors, clinical support staff, providers, and business office staff.
- Serve as a proactive liaison to healthcare supervisors and managers to facilitate data engagement by the expanded care teams supporting population health and value-based principles.
- Compile data/information for reports.
- Attend daily patient care (unit) huddles and periodic informational (clinic) huddles, meetings, patient conferences, or planning sessions related to quality assurance, patient care, and other related topics within the clinic.
- Complete all direct and indirect care documentation in a timely manner and ensure patient records are current and complete.
- Act as a patient and family advocate to ensure that services are delivered to meet the needs of patients and families, as well as appropriate use of resources.
- Comply with all regulatory and accreditation standards, including The Joint Commission and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures.
- Participate in quality strategies to evaluate compliance with evidence-based guidelines, standards, and to identify opportunities to improve patient outcomes.
- Perform any other duties as needed to drive the vision, fulfill the mission, and abide by the values of this organization.
- High School diploma or equivalent required.
- Associate's Degree in Health or Human Services related field preferred.
- 3 years working within a community health setting in a healthcare role or a community-based human service agency as a Community Health Worker (CHW), health educator, community outreach worker, or community organizer required.
