About the Role

We are seeking a highly motivated and compassionate Care Coordination Associate to join our team. In this role, you will work directly with clients to understand their unique needs and develop tailored care plans that address their physical, emotional, and social well-being.

Your key responsibilities will include:

  • Engaging with clients to gather information about their health status, medical history, and personal preferences.
  • Developing and implementing care plans that incorporate evidence-based practices and community resources.
  • Fostering collaboration among healthcare providers, family members, and caregivers to ensure effective care coordination.

The successful candidate will be a natural problem-solver with exceptional communication and interpersonal skills. A bachelor's degree in a related field and at least two years of experience in healthcare or social services are preferred.

Estimated Salary: $64,000 - $84,000 per year
