Job Summary:We are seeking a highly skilled and experienced Care Manager RN to join our team. As a key member of our healthcare staff, you will be responsible for assessing patient needs, developing individualized care plans, and collaborating with interdisciplinary teams to coordinate patient care and services.
Key Responsibilities:- Develop and implement comprehensive care plans to ensure optimal health outcomes for patients
- Collaborate with healthcare teams to coordinate patient care and services, including medication management and treatment planning
- Monitor patient progress and adjust care plans as needed to ensure the best possible outcomes
- Minimum 2 years of experience as a Care Manager RN in an acute care setting
- Active WA RN or Compact License required; CCM or ACM certification preferred
- BLS certification required
This is a full-time position with a 5x8 hour shift, with options for a 4x10 hour shift available upon request. Floating within the scope may be required to meet facility needs.
Compensation:$120,000 per year, offering a competitive salary and benefits package
