**Job Overview**

Devoted Care Hospice Inc is seeking a highly skilled and compassionate Clinical Liaison to join our team. As a Clinical Liaison, you will play a crucial role in building relationships with physicians, clinical staff, and patients to provide excellent care and support.

The estimated salary for this position is $65,000 - $85,000 per year, depending on experience and qualifications.

About the Role

  • Act as a partner between the community and Devoted Care Hospice Inc by seeking out referral opportunities and relationships.
  • Ensure collaboration of care, coordinate discharge planning, and maintain excellent communication.
  • Develop relationships with physicians and clinical staff to ensure seamless care transitions.

Requirements

  • Unrestricted LVN/RN license in the State of Texas.
  • Minimum 2 years of experience in hospice, medical surgical, critical care or oncology nursing.
  • Excellent communication and organizational skills.
  • Hospice experience preferred.
  • Must maintain current CPR certification.
  • Must hold and maintain a valid Texas driver's license and automobile insurance.
  • TB status must be current.
