We are seeking a highly skilled Registered Dietitian to join our team at Peninsula Community Health Services. As a Registered Dietitian, you will play a critical role in providing comprehensive nutrition services to patients with chronic diseases and those at risk of developing chronic diseases.
Key Responsibilities- Develop and Implement Nutrition Care Plans: Collaborate with healthcare providers to develop and implement personalized nutrition care plans for patients with chronic diseases.
- Consult with Providers: Communicate with healthcare providers to ensure that patients receive comprehensive care and that all relevant information is shared.
- Work with the Patient Care Team: Collaborate with the patient care team to identify high-risk areas and develop strategies to address them, including obesity, diabetes, and special medical populations.
- Provide Group Nutrition Education: Develop and deliver group nutrition education classes to patients and their families.
- Participate in Protocol Development: Contribute to the development and implementation of protocols for nutrition services.
- Electronic Health Record Management: Familiarity with electronic health record systems, such as Athena.
- Supervisory Responsibilities: No direct reports.
- Registered Dietitian in Washington State: Must be a registered dietitian in good standing with the Washington State Department of Health.
- Board Certification: Board certification in a relevant area, such as IBLCE or CDE, is preferred.
- CPR Certification: Must hold a current CPR certification.
- Valid Driver's License: Must possess a valid driver's license.
