Kitsap Mental Health Services (KMHS) is seeking a dedicated Vocational Specialist Care Coordinator to join our PACT team. The PACT team provides services to clients most impacted by mental health disorders, such as schizophrenia and schizoaffective disorders.
Key Responsibilities:- Plan and provide work-related supportive services, including symptom management, grooming assistance, and transportation to community resources.
- Provide ongoing support for job retention and educational programs, assisting clients in managing symptoms.
- Offer onsite job coaching or hands-on assistance to clients in navigating educational facilities and resources.
- Engage and motivate clients to explore their interests and aptitudes, utilizing interest inventories or assessment tools.
- Participate in the PACT Comprehensive Assessment process, assessing client status in education and employment.
- Document client progress in a timely manner, ensuring relevance to treatment plan goals.
- Participate in daily staff meetings and schedule treatment planning review meetings for assigned clients.
- Support clients in reducing or abstaining from substance use, employing a harm reduction approach.
- Use situational awareness to evaluate and reduce physical risk factors in potentially unsafe environments.
- Coordinate with team members for crisis intervention, communicating with Mobile Crisis Outreach Team and law enforcement officials when necessary.
- Assist clients in building skills for independent living, including shopping, cooking, cleaning, personal hygiene, budgeting, socialization, and accessing transportation.
Minimum Qualifications:
- Bachelor's Degree in Psychology, social services, or behavioral health field.
- Entry-level experience (no prior related work experience).
- Valid driver's license and reliable transportation.
- Agency Affiliated Counselor Registration.
Preferred Qualifications:
- Experience providing vocational rehabilitation-supported employment or workforce re-entry services to individuals experiencing mental health disorders.
